Additional Notes:
- If a Cream medication is prescribed instead of Pills/Capsules form, there is an additional charge of $79 per medication.
- Membership includes Medication, Consultation, ongoing monitoring, and Follow-up blood work if indicated.
Billing & Payment
- Membership fees are auto-charged on the 1st of every month.
- Membership is non-refundable.
- To cancel, written notice must be submitted prior to the 20th of the month to stop billing for the following month.
